Shakes or Wobbles problem

This is driving me up the wall!!! (close to the point where i actually just want to put it into a tree!!
This car has been nothing but a pain in the arse since i bought it 4 years ago Evil or Very Mad
When i set off in first gear the car starts juddering/kangarooing really badly, its like there's a loss of power when i get into 1st gear and accelerate.
Does anyone have any ideas what could be wrong with the pile of crap?
