Slight vibration when idling in neutral, more heavier when in drive or reverse. Sometimes slight, other times more intense--intensity is intermittent.

I'm not delusional, it's close to a 30 years old. I've owned it for 9 years and put 120k on it myself and am fanatical OCD when it comes to its care and maintenance. The engine is very strong--smooth running. Very recent plug replacement, old plugs showed no oil burn, though thought they looked a little lean.

Smoke tested -- negative leaks
Standard motor mount test indicates mounts are good, though, honestly, I can't but think it's the mounts (hydraulic) even though they passed the rev-on-brake test.
Recent fuel pump replaced.
Recent (1 year) harmonic balancer replaced
Power steering pump solid (relatively new), not a whisper of noise, no issues.
Very recent belt pully and belt tensioner replaced.

A lotta smart people scratching their heads.

Though have yet to get a full OBD2 diagnostic scan.
Ideas?

Yep, that is what I would do first, lets run the codes to see if any are present. This video will help.

Please go over this guide and get back to us. Also, if you can upload a short video of the engine idling it will help us see what is going on. Ken

Right. Though not throwing a check engine light. And as I said, I'm not delusional, I just turned 200k miles on it. And until recently was purring like a cat and running like a cheetah. No loss in power, still pulling 25-27 mpg on freeway. Shifts smooth as it should. Im an otr truck driver, so will be a few days before I could make video.

No problem, if the milage is okay then then engine sounds good. Could it be like a exhaust system rattle?

No. Nothing like that. And exhaust is solid since I just replaced a busted bracket. All indications point toward mounts. But standard brake-rev test shows no excess movement.

The engine mounts can still be collapsed. Have you checked the transmission mount?

A shop did and said all looked good.though if I do motor mounts, solid rubber vs, hydraulic, I'd do the tranny mounts, too. Has polyurethane now, but have rubber mount I'd put on.

If the engine is running smoothly then replace the engine mounts with rubber.
