Transmission/clutch problems, every gear grinding/hard shift

Just out of nowhere it's hard to put into every gear or grinds unless I'm going the exact speed/rpm for that gear. For example will not go bro first unless moving 1-9 mph. And will not go out of gear unless I'm almost completely stopped. I usually start in first and sorta rolls a little bit. Had to do this to get it back home. I pulled over too check out fluid, and clutch pedal. Too reverse I also had to start in reverse. I had to float/rev/RPM match every gear to get back home to park it. Please any information will help me out on what route I should take.

Hi:

It sounds like the clutch isn't disengaging. The truck has a hydraulic system for the clutch. There is a clutch fluid reservoir on the firewall. Have you checked to make sure it isn't low? Is there any evidence of a leak?

This system has a clutch master cylinder and a slave cylinder located on the transmission. Check for leaks at both locations and let me know what you find.
