transmission not shifting into fifth gear or reverse

This does sound mechanical, this really needs to be inspected first hand by a trained transmission tech. they can do a series of on car tests that may show the problem. if this is the original transmission, it may just be worn out as it is now twenty years old. Get on the phone and see what a report will cost, this may be the best place to start.
