Is this a blown head gasket?

I have a 92 Toyota Camry with 221K miles. While driving steam started shooting from a crack in my anti-freeze reservior bottle which was also bubbling and at the same time steam discharged from a small crack in the radiator. Then after further driving I heard a rattling in the engine followed by the fearful "Check-Engine" light. Any thoughts of what the problem could be?
