2001 Toyota 4Runner Engine Hesitation

My Toyota 4runner has developed a significant engine hesitation when I apply the throttle. The Car starts fine, and idles fine but once you go to give it gas the engine hesitates for about a second and then goes. Engine acceleration is not as smooth and strong at lower rpms (1000-3000rpm) once the car finally takes off. However above 3000 the engine starts to perform wonderfully. If I take off very slowly and apply very little throttle, there does not seem to be as much of a hesitation. I have a clean air filter in it, I removed the air intake boot and cleaned the throttle body/butterfly with Seafoam cleaner, and ran a bottle of injector cleaner in the gas. Nothing has helped. It is to the point where it is dangerous due to lack of engine response. This problem will happen without engine load i.e. transmission in neutral and on the road. Please HELP!!! Thank you.

Is the check engine light showing while driving?

I would suggest testing the throttle position sensor and O2 sensor. Check for vacumn leakages.
