Engine idles fine and takes throttle smooth as long as its not in gear. I can rev it all the way up and it doesn't skip a beat. In first gear I don't notice much, but second gear starts to lurch/misfire at about 2500 RPM and its noticeable in all gears on up. The problem is highly sporadic as well. Sometimes it comes and goes daily and sometimes its a problem for a couple days and then runs great for a week or more. And of course everytime I've taken it to my mechanic its run perfectly...

And a quick side question: on my 4Runner, do I have to drop out the gas tank to replace the fuel pump?? And if I have to is there any reason to not cut a hole in the floor of the truck and make an access panel instead of dropping the tank out?

First off, yes you do have to drop the fuel tank to replace the fuel pump. Cutting a hole would allow a lot of extra road noise to enter the vehicle, along with the weather elements. Also, you still may not be able to remove the pump this way because of wires and harnesses that may not be reachable without dropping tank.. It is up to you on doing this.

For your initial problem you are having, you may just need a tune up. The plugs could be starting to fail slightly or there may be a filter that is clogged up a bit causing surges as well.
