2000 Subaru Forester timing belt failure

timing belt was broken when tension assembly holder bolt loosened and sheared off.
It appears that the spacer washer between the timing belt tensioner and cover assembly was left out when timing belt was changed. Could this have caused the bolt to loosen?

It sure could cause premature failure if it has altered the alignment of the belt.
