Slave cylinder and throw out bearing replacement

I am needing a slave cylinder and throw out bearing for my car listed above gt model. All the slave cylinders I see have 2 holes at the bottom where you connect it, but I am needing one that has 4 holes at the bottom where it connects. where can I get that? Down below is a picture of what I need but instead of just having 2 holes I need 4 as you can see in the picture.
