2006 Saturn Vue Vibration while braking

About 6 months ago I was getting heavy vibrations while braking so I took the car in and had the brakes and rotors replaces. About a month later it came back so I went in and had the tires rotated. About a month later it came back yet again and is getting very bad. It only happens while braking from about 35 to 30 mph. After I slow down below 30 it stops and if I am slowing down but still above 35 it doesnt happen. Please help.

is the vibration in the front while braking and can u also feel it in the steering wheel?if so its ur front rotors warped.also do u have drum brakes in the rear if u do then they might be out of adjustment if they are then all the braking is being done in the front causing the rotors to warp.another reason is the way u drive i have seen alot of people the way they drive warp the rotors over and over again.
