Replacing a passenger side mirror?

Unfortunately, it does appear that the door panel needs to come off because there is a screw holding that trim piece in place under the panel.

As for that mirror knob, yes it should just unscrew off the shaft.

As was stated above the trim panel must come off to access the screw, and the studs that hold the mirror on as well. The knob will usually fit through the trim plate on them, but you need to loosen the set screw almost all the way out as the slot in the mirror control that it grabs in for alignment has a step in it to help retain it. Be gentle with the trim as that plastic really likes to break easily.

Okay, where do I start to pull out the door panel and can I just pull it out enough to get at the screw at the top? A picture of the exact point panel come out would be appreciated. Thanks

The door panel is not hard to remove, and I would not try to pull it away just enough because it will most likely break some clips or plastic pieces.

Yes, you do not want to pry on the panel to access the screw, it will crack at the least. You need to remove it because you may get the trim screw out, but you still need to access the other studs that retain the mirror.
Remove all of the screws and trim indicated in the instructions, then starting at the bottom pull out on the trim panel just enough to reach in the gap with a putty knife or similar tool to release the clips from the back of the panel. With those free you now push in and up on the trim panel where it nears the glass, be sure the window is fully down as the trim needs that space to move into to release the upper clips. Once the panel is off you have access to the screws on the smaller trim section. Remove it and the small access cap and you will see the mirror studs under it inside the door.

There are a ton of options out there. Just do a Google search and you should pull one up. I found this one on Ebay for a pretty good price.

91 K-cars don't even exist around here anymore. I just ran the OE number, and they showed a few local stores that it could be shipped to, AutoZone and Advance were listed as stores that you could order it through as well.
The OE part number is CH1321125 if you wanted to look for that. Shipping is starting to get really bad lately, parts supplies are getting to be difficult as well, especially if it's anything that is coming into California now that they banned many trucks from operating in the state. Even dealers are going to the auctions and buying cars just to strip parts to repair ones on the lot.
