Very rough gear changing

Thanks for your help in advance. Always a very rough gear changing every time when changing gears from parking to either drive or reverse only. other then that car runs great . Have not done a transmission fluid change for years . Think if I do a fluid change will solve the problem..

Good place to start and do the filter as well. With the pan off check for any metal debris in the pan also any signs or brass start here.
