Pontiac Transport error code 44

I have a 1995 Pontiac Transport 3.1 liter with approximately 200k miles and when I put the code reader on it, I get fault code 44 - lean exhaust mixture. The repair manual I have doesn't appear to have a solution to this so was hoping you might have an idea of what I need to repair on the vehicle.

Quick update - Found a resource online that stated a clogged gas filter, water in the gas or a vacuum leak could be the cause. I changed out the fuel filter, drained the tank and put in fresh gas and checked for vacuum leaks to no avail. One of the other things is said it could be in the oxygen sensor but I'm not to sure where its located at on my vehicle.

Could someone reply with a link or picture that will show me where this is located at?

The o2 sensor is in the exhaust pipe, at the rear of engine.Be sure to go after the one closest to the engine, the other measures the efiiciency of the cat converter. Might be a good idea to take a look at the Mass Airflow Meter, they go out of range and cause rich and lean codes

Well, found and replaced the O2 sensor and it still hasn't corrected the issue, guess I'll be replacing the MAF sensor tonight to see if that resolves the issue. In case that is not it, is there anything else I can look at that might be causing this issue?

Ok, I have replaced the MAF sensor, oxygen sensor, fuel filter, PCV valve, air filter, replcaed the gas with new, and I still get this same error code 44.

Is it possible that the fuel injector filters need to be replaced or might there be some other cause for this that I am overlooking?
