1999 Plymouth Voyager windshield wiper 'timing' problems

Several weeks ago one of my windshield wipers would not turn, the other was fine. I called the dealer and they said to tighten the bolt at the base of the arm. I did this, and it was fine for about a day- now, not only am I still having problems getting this to turn, but the timing seems to be off and this wiper is piercing the passenger side wiper and ripping the blade aprt. i've fiddled with the bolt and have replaced the passenger wiper 2x - any other suggestions?

Shut wipers off
and loosen the nut at the bottom of wiper arm

adjust the wipers arms
you may have to do couple of times
