Code P1391, engine hesitation and tachometer drop when shifting?

Engine hesitation and tach drop when shifting between 20 and 25 MPH and 40-45 MPH. Is it camshaft or crankshaft or both or neither? Runs good otherwise. Starts, no backfire. Check engine light comes on but is off the next morning.

I would do the crankshaft sensor first, here is the location of the sensor so you can swap it out, clear the codes once completed.

The P1391 code on your 2001 Dodge Caravan indicates an "Intermittent Loss of CMP (Camshaft Position Sensor) or CKP (Crankshaft Position Sensor) Signal."

A mechanic told me he thinks the transmission is going out and sensors won't help. What's your opinion/advice?

That could be, but the code is for the crankshaft sensor. You might have two problems.
