When I reach around 40 mph the car slips out of gear if i tap the brake, and when i reach 60 mph it will not shift into a higher gear to keep going. Also, it runs at 4,500 rpm's after 40 mph.

How is the power of the engine? because it sounds like you have a clogged catalytic converter. Can you please shoot a quick video with your phone so we can hear the noise? that would be great. You can upload it here with your response. Have you checked the fluid level?
