My car jerks..

So I have a Hyundai Elantra 05 and when I start to accelerate or maybe a little bit after as in getting faster my car jerks and hard! Like it jolts!! It's scared me every time please help!

scan for codes and check fuel pressure first then check trans fluid if you can as most newer cars you can't a pro has to do it.
