Manifold codes come back almost immediately?

Am almost sure this is common, but i never fixed one before. Sometimes a bit of a rough idle when first started then fine. Delete codes and they come back almost immediately after i restart. Codes attaches.

The system to lean and the oxygen sensor voltage low is telling us the MAF sensor is bad, I would replace it will a new one and then clear the codes. How does the engine sound while idling, can you upload a short video? Also, does the engine have an exhaust leak? Here is the location of the MAF sensor. Check out the images (below). Let us know how it goes.

Changed it and it seemed better. But then after I drove it a few miles there is still a hesitation, and rough idle RPM goes up and down slightly. No check engine light but i scanned it and the o2 sensor code still there. Disconnecting the mad sensor makes it better but the fan stays on high.

Is it possible a valve cover gasket leaking can cause such issue?

I don't think so, with the engine running remove the oil fill cap to see if there are pulses. This can tell us one of the pistons is broken.

No pulses. Engine is smooth otherwise, no major engine issues or noises, drives smooth after.

Did you change out the MAF sensor? Also, what are the Short trim readings?
