When driving engine stutters and shuts off

First, when the engine is started there is a loud grinding, rumbling and screeching noise and then it settles.
when the car is in action it stutters and most recently the engine cuts out.
what is the likely cost of all this car trouble?

First, the list in the picture doesn't include anything that would be causing this type of problem. When you say you hear a screeching when first started, does it sound like a belt? Is it possible for you to record the sound and upload it for me to hear?
