Transmission keeps popping out of fourth gear

Hi, I'm Dan.

It sounds like it is most likely an internal issue inside the transmission. Something like a worn shift fork or detent ball. I've attached a form from Toyota that shows the likely areas to check. I've also attached an exploded view of the transmissions internal shifting mechanisms.
