77 cj5 w/304 timing problems

I have a 77 cj5 with original 304 with 77,000 +- miles
engine jumped time, removed cover and found cam gear jumped off the keyway, replaced with new gears, chain etc. problem is while jeep is under a load, taking off, accelerating etc, it spits and sputters.

is it possible for cam to be out of time with the crank?

new items include:
timing gears and chain - made sure they were dot to dot several times
new timing gears and chain (twice replaced)
new dist with all internal components, cap and rotor
new wires and plugs
new carb
new coil
new fuel pump and filter
what am i missing or done wrong????????????

Maybe its in the exhaust stroke instead on the compression BTDC 0deg. Did u put engine on TDC before setting all the marks as illustrated. If not sorry to say you're going back in again
