1996 Honda Accord Clutch

I have been having issues getting the car in and out of gear. First off the clutch pedal is very soft and without a lot of presure. Some times its hard to get the car in gear. Other times its smooth. Does it sound like the clutch is going out or do I need a new clutch cable? Please help
