Harmonic balancer issues

I have a harmonic balancer that keeps failing (on the fourth in as many weeks).

So far, I have tested all pulleys, and with the most recent replacement, I replaced the tension/idler pulley along with the harmonic. I am afraid it go again, and before it does I was looking for ideas on what else might cause it.

My last thought is crankshaft, but there are not any other issues until the harmonic fails and the belt slips off.

Hi and thanks for using 2CarPros.com.

I am not sure what is failing other than it may be coming loose. If that is the case, I recommend using Loctite on the threads. Also, when tightening the bolt, make sure it is getting tight enough. It requires a special tool to hold the dampener when tightening it. I have attached the install directions for your review. The attached pictures correlate with these directions.

INSTALLATION

Special Tools:
1. Apply a light coating of engine oil to the front crankshaft seal surface (1).
2. Align the crankshaft vibration damper (1) to the flywheel key on the crankshaft and install the damper. Seat the damper on the crankshaft sprocket.
3. Install and hand tighten the crankshaft vibration damper bolt (2).
4. Hold the crankshaft vibration damper (1) with Vibration Damper Holder (2) and tighten the crankshaft vibration damper bolt to 40 Nm + 105° (30 ft. lbs. + 105°).
5. Install the accessory drive belt See: Drive Belt > Removal and Replacement > Serpentine Accessory Drive Belt - Installation.
