2003 Ford Taurus Serpentine Belt

I have had to replace the belt 3 times in less than 4 months. Then at the last one, I put the belt on and cranked it up. The belt slips off and won't stay on. The alternater was bought used a couple of months ago. Each time I put on a new belt, it gets shredded. What could be the problem here?

There is a problem with pulley alignment somewhere. Align all the pulleys by eye and look for one not aligned with the rest or any pulley that leans, even slightly, when the pressure of the belt is applied.
If you install the belt and have someone else start it, you should be able to see which pulley it migrates off first.
