Transmission problem

hey i have '92 ford eb fairmont and there seemes to be a problem with the transmission when i have it in drive it doesnt seem to shift gears after the 1st gear. it you have your foot flat on the pedal it doesnt rev out and doesnt go to the next gear and it only stays on just under 2grand when you have your foot flat on the pedal. but once you have past 60km/h the grand then starts to move up. please help!
