1995 Ford F-450 Antifreeze

What are the probable causes of a leak of antifreeze? I fill the resevoir and within a week it's empty. I'm pretty sure the resevoir is ok. What else could it be?

busted engine block, cracked head gasket and/or head, faulty radiator hose(s), failing water pump, busted radiator etc.... check engine oil to see if it looks like coolant is mixing into it. also check for visible signs of leaks i.e. smoking out exhaust or wet spots in engine compartment.
