'84 F150 4x4 5.0 4spd Transmission Saga Continues

This is my third posting about this truck and my tranny problems. The truck now will only drive in 4L. In 2H and 4H I just hear this spinning/grinding noise matching the engine speed.

I had to drive 2 miles home at 5mph last weekend. Not good!

Any help? Input shaft? Throw out bearing?

Also, where is the model number cast or stamped on this tranny? I woudl like to be able to clearly identify what model I am dealing with.
