Headlight Assembly clouding up

I have a 2001 Ford Escort automatic 4cyl with 87000miles and the headlight assembly is clouding up. How do I remove the whole assembly to either clean or replace the headlight assembly due to it clouding over?

I use a power buffer and "Soft Scrub w/ bleach" on mine. Once you start, it becomes a thing you need to do every month or so...
