Engine Performance problem

my escort with 170,000 plus miles has started reving up to 2to 4,000 RPM while idling or between gears when clutch is engaged I replaced idle control valve and worn or broken vacuum hoses but that didnt help so i took it to dealer where they replaced a broken sensor (coolant temp I think) which seemed to fix it for 2 weeks then it started same stuff again.It seems like if I shut it off and restart 3 or 4 times it will idle normally and run fine until I have to turn it offagain.Also cooling fan only seems to work if I keep the A/C on.I also used a sensor tester/code scanner and it showed no trouble codes,I'm stumped and could use some help.
