what causes error codes P0172 and P0401? What is the meaning of them and how to fix them?

Code P0172 - System Too Rich (Bank 1)
Code P0401 - Exhaust Gas Recirculation (EGR) Flow Insufficient
The system is running too rich, this is usually because of an exhaust leak up by the exhaust manifolds or a leaky fuel injector, how is the engine running? Please go over this guide which will help by showing us what can cause this issue.
