Dirty fuel injectors (cleaning the injectors often fixes this).
Bad MAP (manifold absolute pressure) sensor
Bad TPS (throttle position) sensor
Bad or dirty MAF (mass airflow) sensor
Low fuel pressure (leaky fuel pressure regulator or weak fuel pump)
Vacuum leaks (intake manifold, vacuum hoses, throttle body, EGR valve)
Bad gasoline (fuel contaminated with water or too much alcohol)

Sometimes, what feels like a hesitation is actually ignition misfire rather than lean misfire. The causes of ignition misfire may include:

Dirty or worn spark plugs
Bad plug wires
Weak ignition coil
Wet plug wires

Thank you very much for analyzing the problem in my Dodge Dakota pick up. I have been trying for about a year to fid what is wrong with it. I got various answers from the garages here, but nothing helped. The first garage replaced the electical points plugs and wires etc. The next checked it over and charged $100 but wouldn't fix it. They said it was too old to fix (1996). The next one said his computer indicated a leaky manifold gasket. That didn't help. Later on I found the hose from the crank case had a big split in it. I replaced that and it ran better. Then he said the fuel filter was partially stopped up. So he replaced the fuel filter, fuel pressure regulator and that didn't fix it. So $1500 later I was getting desperate and started looking online. How much trouble is installing an E.C.M.?
