1996 Dodge Caravan shakes when appling the breaks

My car shakes only when I break at high speeds 50mph or higher.If you apply the breaks slow and stop and reapply its not as bad but still there. The breaks and rotors have been changed. Any Ideas?

Is the steering that shake or the van
if the steering check front end and rotors in front
if the van that shake check rear brakes out of round
