2001 Chrysler Concorde Wondering?

Hi,
I was wondering if you guys can answer a question for me. For the past couple days my car is beginning to shift gears on my car while driving. I can go 45 mph , and the all of a sudden it would kick in and my rpms would go up. Wondering what can be wrong.
