1999 Chrysler Concorde car jerks at certain speeds

Most of the time the car starts and idles fine.
When I get to about 20-25 miles per hour it starts to
jerk as if the engine and transmission are being disconnected momentarily. As soon as I pass 30-32 mph it runs fine until I get up to 42-43 mph same thing up to 48-50 mph, all is fine again, until I get up to 62-63 mph it seems to be even worse than the lower speeds until I reach approx 68-69 where I set the cruise control and the rest of my comute ( about 45 miles ) everything is fine. this is a daily occurence for more than 3 months. please help

You might have already fixed your car, but I had the same problems, almost exactly. The problem with my car was the CRANK SHAFT SENSOR. My car runs great now.
