why would the fan belt come loose? Is it the fan belt or the pulley?

You probably have a bad pulley or a broken belt tensioner. A bad belt or incorrect belt size could cause it also.

I found out that the main engine pulley has moved outward and thats why the belt is falling off the other pulleys. The main engine pulley that is connected to the cranck shaft is moved out about two inches. Why is this. Is a bearing out? or does it just need to be fitted again and bolted back up? I have a new fan belt and the spring pulley is working just fine it's that main engine pulley that is making my car not run.

sounds like you have a bad harmonic balance. the pulley is actually bonded in rubber to part that is bolted to the crank. there is no fixing it, you must replace the harmonic balancer.
