1998 Chevy Truck surges

I have a 1998 chevy pickup and it started to stumble and hessitate if you hold it steady. it will surge going down the road. i have replaced the fuel filter but did not fix it. i cleaned the IAC valve and checked vaccum lines for leakes but no change. thanks for the help .

Sounds like a sticking exaust valve or a failing head gasket.

i think i got it figured it out. i suspected bad gas and i poured some b12 in it and immeditely seen a difference but still some hesitation. i am gonna try to flush the fuel system and hopefully that will fix it for good. thanks everyone for the help.
