My steering wheel shakes between 45mph and up. What causes this and how much would it cost to fix.

Hi:
It could be many things. First, have the front tire balance checked. If no luck with that, have the alignment checked and front end steering components. Finally, if it only happens when you use the brakes, replace the front rotors.
