5.7L 4x4 changed plugs, wires, dist cap, rotor, replaced fuel injectors. use to get 16 to 17 Hiway MPG. this started at ~ 90K miles. Seems to idle ok. Now milage is ~12.3 to 12.8, I drive like an old man, easy excelleration and speeds at 55. Using Regular or medium grade, makes very little difference.

it could be your o2 sensors. may have two, one on each manifold, three the prvious two and one befor cat converter or maybe even four with one after the cat, but i bet just the two. strong Emphasis on the word could, but i have a feeling it just may be your cause
