1992 Chevy Blazer warms up, dies at idle and or when drivin

my 92 blazer full size will start fine when cold and drive good until its warm, after that if i stop accelerating it dies and does not want to stay re-started. i have to hold the gas pedal down and shift into drive from neutral when at red lights or stop signs in order to hopefully stay running. doesnt always like to restart after dieing out either. already replced the Idle speed censor (think thats what it is) This 350 engine is TBI. What the heck causes this issue?

is there any other trouble codes? what is your temp sensor reading when it is warm? You may also want to check for a vaccum leak in the engine?
