What could cause the fan belt to come off and where in Toronto would be a good place (affordable) to fix the problem?

Causes:
1. Over-stretched belt.
2. Bad pulley (one of them).
3. Faulty tensioner assembly.
I wish I can help with shops in Toronto. Never been there. My suggestion is to go to a nationally recognized chain, so you can exercise the warranty (if offered, and when needed) almost everywhere the chain has shops.
