1998 BMW 740 V8 Two Wheel Drive Automatic 180K miles

I have a 740I V8 4.4L Check Engine light on got a Code 26 & 27 Fuel Trim Lean Maximum? What does it mean and how can I repair it?

It means the engine management is trying to compensate for a possible vacuum leak. Has the PCV plate ever been replaced?
