Transmission problem

When shifting from "park" to "drive" there is an immediate loud grinding noise from the general transmission area. It does not happen when the car is in reverse, just drive. My wife drove it for a few blocks while making this noise and said that it shifts through the gears but still makes the loud grinding. She says that it started with a thump, and then the grinding started. Sorry for the second hand question but I am currently on duty in Iraq and am relaying this problem from my wife at home in San Diego.
