Blinking 'D' light, very hard gear shift

First of all I would like to thank you.

Secondly, this causes jerk to the complete car. I thought it was transmission oil, it is same even after changing the oil. If accelerate it too much the car stops and shows check engine light, check oil and battery light all three together.

Hi and welcome to 2CarPros. We do not have the Honda Jazz here in the USA but the symptoms you show may indicate a low engine oil level. Most manufactures use oil pressure to control engine timing. If the oil level is low it can cause timing issues. Also low charging output will cause it to act up. Check your charging system as well. Here is a link that explains the basics:

Also if you can, check for trouble coded using a scan tool. If you have codes then post them here to see if we can look it up.
