automatic transmission oil in the water system and vice versa

I have ford explorer 2006 at my garage .. one day the costumer came to my garage and there was a/t oil in the water system soo i checked it and replaced the water radiator since the cooler is part of the water radiator .. so now after only several months the car is back having the same problem ... there is oil un the water system and also water in the transmission .. is there anything other than the cooler which causes this ?! Thank you

No, that's pretty much it. The only place trans fluid can come in contact with coolant is the radiator.
