Is it possible for an oil filter to fall off 3 weeks after an oil change?

No. It has to be unscrewed many revolutions. After just one or two revolutions you would have had a severe oil leak. All of the oil would have pumped onto the ground within a few seconds.

It IS possible for them to be not tightened enough and begin to leak slowly. Your oil pressure warning light would have turned on and you would have stopped the engine before serious damage occurred.

Sometimes in our haste we neglect to notice the rubber seal from the old filter is stuck to the engine and we put the new filter over the top of it. That's called "double-gasketing". The old seal will usually blow out as soon as the engine is started and dump oil all over the floor, so it gets caught right away.
