car jerks and shuts down

after driving for 30 minutes, cars slows down and cuts off. when starts up it jerks even if i floor the gas. engine light comes on then eventually it does not start untill about 5 hours

It sounds like an issue with the crank sensor. Have you had the computer scanned for codes? If the check engine light is on, there will be a trouble code stored that will identify where the problem is coming from.
