What are these pulleys called? I know one is the harmonic balancer...which is what?

My belt keep slipping off and we have replaced the tensioner and idler pulleys. Once we put a new belt on it will ride for about a week then it starts slipping at the tensioner pulley. We think we need to replace the harmonic balancer. My balancer has two pulleys on it like the picture I attached. When I go to purchase the harmonic balancer it only has one pulley on it. What do I need to get?
