The next suspect would be a leaking cylinder head gasket. A quick chemical test at the radiator will identify if combustion gases are getting into the cooling system. When those gases pool under the thermostat, that will prevent it from opening. Thermostats must be hit with hot liquid to open. Hot air won't do it.

The test involves drawing air from the radiator through a glass cylinder with two chambers partially-filled with a special dark blue liquid. If combustion gases are present, the liquid will turn bright yellow.
