Why does engine hesitates and rattles when accelerating under 55 mph

The engine on my 2001 Chrysler Town and Country van rattles when accelerating and seems to miss unless I accelerate to a higher speed. This usually happens when driving/accelerating between 30mph to 55 mph. It does not rattle when idling. This started happening after I filled my gas tank up at Ayerco gas station, the van started loosing acceleration power soon thereafter. We have added Lucas fuel injection cleaner, changed the EGR Valve, fuel filter,spark plugs and spark plug wires. This did help a little, the acceleration problems went away a lot, but the engine still seams to be missing when trying to accelerate, and it still rattles slightly when accelerating, but not as bad. Some people think it is engine mounts or the tie rods. I drove in 3rd gear for a little while today and it did not miss or rattle much. I am planning on going to Firestone or Sears Auto Center to get a diagnostics, but I just want your opinions, because there are some shady auto repair centers/mechanics out here. I would appreciate any help.

scan' for codes check fuel pressure and check for a vacuum leak first. vacuum leak like a hole in hose, hose off,etc.
