2000 Ford Winstar LS sputters/chugs when idling or accelerating

When accelerating the van chugs / hesitates. Also when sitting at a stop light it feels like it wants to accelerate. What could this be? Would this have anything to do w/air somewhere?

scan for codes and check fuel pressure first then try cleaning throttle plate with choke cleaner on both sides as well as iac hole.
